1) Section A) Add your name, DOB, nationality, country of birth and gender details
2) Section B) Indicate the licence you are applying for i.e door supervisor

1) Complete Section C)
Counter-signatory identity and declaration (UK passport holders only).
2) Complete Section D) Applicant verification and contact information (email, telephone, N.I number, passport or driving licence number)

2) Complete Section I) Application declaration and consent. Read carefully and sign (sign inside the box and date your signature).

The form includes some extra sections for additional information if there is not enough room on the relevant section i.e. previous addresses, medical history, etc.
